When completing the Apprentices Details Form, you will be asked to upload documents to evidence the following:
- Proof of Identity and Right to Work
- Proof of Address
Please read the following guidance on the types of evidence we will accept and ensure that you have these ready to upload before starting to complete the form.
Please note that uploaded files should be in one of the following file formats and should be no larger than 4MB:
- JPG/JPEG
- PNG
- TIFF
Proof of Identity and Right to Work
- If you are a UK, EU or EEA Citizen, you will need to upload the document listed in Option 1 or all documents listed in Option 2.
- If you are a non-EEA citizen, you will need to upload the relevant documents listed in Option 3.
Option 1: UK, EU or EEA Passport (in date and not expired). When uploading a passport, this is the page you should supply:
Option 2: All documents listed below:
- Birth certificate
- National Insurance number evidence – copy of National Insurance card, letter, payslip (less than 3 months old) or copy of P45 / P60
- Passport photograph - signed and confirmed as a true likeness by the Branch Manager or Workplace Training Supervisor
Option 3: Non UK, EU or EEA Passport (in date and not expired) with accompanying right to work evidence.
- Passport to be submitted with all relevant right to work documentation, one or more of the below:
- Passport pages to show endorsement that you are allowed to stay in the UK and are currently allowed to undertake work in the UK
- Current Biometric Residence Permit
- Current Residence Card
- Current Immigration Status Document containing a photograph issued by the Home Office with a valid endorsement indicating that you are allowed to stay in the UK and currently allowed to undertake work in the UK, together with an official document stating your permanent National Insurance number and name issued by a government agency or employer
Proof of Address
You will need to upload documents relating to one of the following options:
Option 1: Driving Licence - full or provisional
Option 2: Utility or Council Tax Bill – less than 3 months old

Option 3: If no bills are in your name, we can accept a copy of a bill which is less than 3 months old bearing the name of the emergency contact / next of kin as detailed on your Commitment Statement with a signed and dated statement from this person confirming that you live at the same address.
