HLP Complete Package - Enrolment Form

Welcome to the online enrolment form for the HLP Complete Package. This form allows you to enrol one "Health Leader" onto the Leadership for Healthy Living Pharmacies Course and one "Health Champion" onto the RSPH Level 2 Award in Understanding Health Improvement (Health Champion) Course. The "Health Leader" will also be given access to the Buttercups HLP Toolkit. 

Please note that the Health Champion and Health Leader should be two different members of the pharmacy team.
